사업내용 | 중저소득국가에서 선천성 심장질환을 갖고 있는 환자들이 해외 병원을 찾거나 외국 의료진에 의존하지 않고, 자국 의료진에게 수술 및 치료를 받을 수 있도록 다양한 방식의 교육(현지 연수, 초청 연수, 온라인 연수)을 통해 현지 의료진의 의료 역량 강화를 도모하고 있습니다. 1. Background Congenital heart disease (CHD) is the most common type of congenital anomaly and the second leading cause of death in infancy and childhood, particularly in developing countries. This is further worsen by limited human resources and infrastructure in the health sector. JWLee Center for Global Medicine led by Professor Woong-Han Kim has cooperated with a number of middle and low income countries to strengthen the capacity of local medical staffs and to promote team-based approach in congenital heart surgery and patient care. The program includes dissemination of medical knowledge and hands-on experience to the local medical staff, from diagnosis, surgery, to post-op care. 2. Cooperating organizations (2018) AKFA Medline, JWLee center, Seoul National University Hospital, Noodle Lovers (㈜ 면사랑), Raphael International 3. Activities and Output 1) Surgical intervention/Ultrasound diagnosis/Post-op care Patients with possibility of suffering from CHD are invited for echocardiogram examination during the visit, conducted jointly by cardiologists from Korea and from the local hospital. Then the Uzbek and Korean medical staff work together to classify CHD patients into three categories: surgical operation / cardiac catheterization / follow up. After the selection, Korean team with surgeons, anesthesiologist, scrub nurses and perfusionists and Uzbek local medical staff in the respective departments jointly perform operation. Or those selected for cardiac cath are operated by the Uzbek and Korean cardiologists. After the surgery, post-operative care is given also jointly by Korean and Uzbek PICU nurses. So far, 111 patients have been operated in surgical treatment in Uzbekistan. 2) Medical staff training All of the processes of the ultrasound diagnosis, surgery and post-op care are part of hands-on training. Throughout the whole patient-care process, Korean staff share their knowledge and skills that can be applied in local setting which requires active communication with the local staff. During the program, each department (cardiologist, perfusionist, ICU nurse, anesthesiologist) gives lecture on CHD patient care, and there is a case-review conference at the end of the mission, to review the cases of patients who have been diagnosed and received surgery during the visit. This session also gives a chance to review the whole process of surgery patient management from diagnosis to post-op care and the follow-up plan. Every participant of the program openly exchanges his or her view and opinion on the patient care. Invitational training is aimed to provide trainees more experiences with more complex cases as well as team-based approaches in surgery. From 2016 to 2017, 1 invitational trainee from Uzbekistan had visited SNUH to learn from and experience highly technical and advanced tertiary healthcare, established within an exemplary healthcare system in a high-resource setting. |
주요활동 | TEAM BASED LEARNING : 의사, 간호사, 수술방, 중환자실 등이 유기적이고 체계적으로 움직일 수 있도록 팀 기반 교육을 진행하고 있습니다. 1) 현지연수 현지 의료진들과의 협업을 통해 초음파 진단, 수술 환자 컨퍼런스, 이론강의, 핸즈온(HANDS-ON) 교육 최신 의학 지식과 기술을 전수하고 각 국가의 상황에 맞게 활용할 수 있도록 교육하고 있습니다. 2) 초청연수 국내에 현지의료진들을 초청하여 수술 및 환자 관리 참관, 그리고 교수진들의 이론강의 제공을 통해 우리나라의 의료 체계를 직·간접적으로 경험하고 이를 자국에 적용할 수 있도록 하고 있습니다. 3) 온라인연수 심장수술과 관련된 8개의 분과 전공과목(소아심장학, 마취통증의학, 흉부외과학, 체외순환학, 수술장 간호학, 중환자 관리학, 소아 중환자 간호학, 영상의학)을 교육하는 비대면 온라인 강의를 진행하고 있습니다. *** 관련 출판논문: Han S, Choi S, Heo J, Park J, Kim WH.
